A rather special cow, named ' The Cow King', always appears in the level. The Secret Cow Level is a small area, full of Hell Bovines: bipedal cattle with polearms.

For example, one must first beat the game (through Act IV in the original game or Act V in the expansion) on Normal difficulty before you can create the Secret Cow Level in a game of Normal difficulty. Players can only create this portal when playing at a difficulty level they have already completed. A red portal will appear in the Act I starting area. To create a portal to the secret level, combine Wirt's Leg and a Tome of Town Portal in the Horadric Cube. The Secret Cow Level, also known as the Moo Moo Farm, can be found in Diablo II (with or without the expansion Diablo II: Lord of Destruction). You could potentially turn what may have been a 3 open socket 15%ed superior archon plate that someone would pay a pretty penny for into a magic archon plate that charsi will take off your hands for that sweet 35k gold. Not to mention there is technically a downside as well when you turn more normal quality items into magic items. You increase your chances of getting a unique item more by going from 0-200%MF than you do by going from 200-1000%. When it comes to unique/set items there are very harsh diminishing returns with MF. Keep in mind that wherever you decide to go, don’t sacrifice your ability to kill too much for MF. Do a google search for D2 area levels and look for alvl 85 areas and find the ones that don’t have too many immunes for your element type. If you want a chance (although still astronomically low) of finding the extremely rare high-level uniques, you need to be in high level areas (the pit, ancient tunnels, mausoleum, chaos sanctuary, baal runs). They can’t drop super high level items, but they’re great for farming up Shako, HoZ, Occy, etc. Mephisto and Andy are better if you want a reasonably easy place to farm on hell for consistent unique/set drops. Runes are completely unaffected by MF, however, so if runes are your goal you want to go for kill speed > MF.

Cows are good for rune drops because you can kill a lot of them in a short amount of time, it’s a great place for monster density.
